Authorization Agreement for Automatic Deposits

I hereby authorize Versivo, Inc. (“Versivo”) to initiate direct deposit, credit entries or debit corrections of all amounts payable to me through the Versivo payment program to the depository institution(s) and account(s) identified below. Versivo allows routing of compensation to a maximum of four (4) accounts.

This authority is to remain in full force and effect until Versivo has received written notification from me of its change or termination or Versivo terminates its direct deposit program or my participation therein.

With respect to payroll deposits, I understand that should I fail to submit complete and accurate timesheets on a timely basis, Versivo may without prior notice terminate my participation in this program and may also deny my reapplication.
